P.I.E. fades on a slower arc than active acne settles. The honest timeline:
- Weeks 4 to 8. Early reduction in the brightness of recent marks. Marks from breakouts that healed within the past few months tend to respond first; older, more entrenched marks take longer.
- Months 2 to 4. Cumulative fading. The frequency of brand-new red marks should also be falling if your acne pattern is settling.
- Months 4 to 6. Most achievable response from topical alone. Marks that remain at this point are typically older, deeper, or in patients with vascular reactivity that responds better to in-office vascular laser treatment than to topical management.
Daily SPF 30 or higher is part of the protocol — sun exposure prolongs P.I.E. and worsens any concurrent PIH. Mineral filters with iron oxides are particularly recommended for patients with concurrent hyperpigmentation. Results vary by individual and depend on consistency of application and concurrent acne control.
